The Kingstanding unit verges between being unprofessional and dangerous. Upon entry, my partner and I could see that a woman (presumably the manager) was storming around in the background huffing and puffing. This was off-putting and unprofessional. I was then informed by the receptionist in an open environment (in front of others and without being asked if I wanted my partner to be involved in the conversation - an absolute basic for any clinical setting) that I would likely require trans vaginal ultrasound rather than trans abdominal. The receptionist said she would “ask” the sonographer if he would consider TAS before trying TVS (surely this is my decision as the paying patient? From both a commercial and consent perspective, her approach was poor). I agreed to TVS for the sake of accuracy and was provided with an unnecessary chaperone. I wasn’t asked if I wanted her to be present, and had I been asked I would have said no on the basis that my partner was with me. The sonographer had poor people skills and did little towards explaining what he would be looking for, how the appointment would work, and what the potential outcomes could be. The bench was ill-fitted and ill-positioned for an early pregnancy scan and I was given very unsympathetic instructions on how to position my legs. While the sonographer did explain he was going to insert the TVS probe, he did not inform me that I could ask him to stop if I experienced discomfort, he did not warn me that I may experience some pressure and cold gel, and he left it to the clearly unqualified chaperone to provide instructions on removing my underwear and covering myself. I only knew to use the paper towels provided to maintain modesty because of my experience of such environments, not because any clear instruction was given. While I could not see the screen for myself, my partner could and he could also see the facial expression of the chaperone. Apparently, her expressions were easy to read and it was clear she could tell something was wrong. If you’re going to use an unqualified receptionist to chaperone, at least train them in the basics of responding to such situations. The sonographer did not communicate what he was doing throughout the procedure, which added to my discomfort. He then announced that it was likely a pregnancy of unknown location and asked “had I ever had cysts” mentioned to me before. I asked him to show me the ultrasound and he vaguely referred to where my endometrium was but gave no information as to the size of the cysts. In my report it has been claimed that I have been offered safetynetting advice. As someone who does provide safety netting advice, I can categorically say that the sonographer failed to do this. I was offered no information on what to do if I showed any signs or symptoms of an ectopic pregnancy nor was advised on where to go if I did show such signs or symptoms. To add insult to injury, the “obstetric report” claims that I was given safety netting advice and provided with emergency contact details on what to do if I have concerning symptoms. Both of these statements are a lie. While I appreciate that the outcome of my pregnancy is entirely outside of WTTW’s control, the competence of their clinicians is not. It could not be more clear that this unit is geared for positive outcomes only and is woefully ill-equipped for poor outcomes — to the extent that the business model is both dangerous and lacking in compassion. It’s only due to my background and ability to interpret NICE CKS guidelines that I now know what steps to take. My advice, at least hire compassionate clinicians, educate your chaperones on decorum, retrain clinicians in basic safety netting that goes beyond “take another pregnancy test and contact a doctor” and provide DHSC-approved leaflets on PUL. Or, stay in your lane that focuses purely on gender reveals and 4D scans rather than coldly and callously capitalising on someone’s early pregnancy anxieties without offering a compassionate service in return.
